The following important forms can be found in this section:

Withdrawal form (for members who wish to take a resignation benefit after leaving office).  The completed form plus supporting documents should be sent to zzpobpen@alexforbes.com 

Retirement form (for members who wish to take a retirement benefit after leaving office).  The completed form plus supporting documents should be sent to zzpobpen@alexforbes.com 

Fund Death Benefit - Expression of Wish (nomination) form

Members are asked to complete these and keep them up-to-date.  The completed, signed form should be returned to the Fund at zzpobpen@alexforbes.com  

 Members are asked to complete these and keep them up-to-date.  The completed, signed form should be given to the Members' Affairs department at your Legislature, for safe-keeping.

Paid-up Withdrawal form (for members who left office earlier, and now wish to take a resignation benefit).  Completed forms should be sent to zzpobpen@alexforbes.com 

Paid-up Retirement form (for members who left office earlier, and are now aged 50+ and wish to take a retirement benefit).  Completed forms should be sent to zzpobpen@alexforbes.com 

Funeral Benefits claim form

The completed form plus supporting documents should be sent to zzpobpen@alexforbes.com 

Death claim form - death of a Fund member (Alexforbes form)

This form should be completed to notify the Fund of the death of a member.  The completed form plus supporting documents should be sent to zzpobpen@alexforbes.com .  In the event of the death of a member, the Trustees will carry out an investigation into the family circumstances (as required by the Pension Funds Act) - please see "Guide to the allocation of death benefits" for more information.  If the member was still in office on the date of death, the Sanlam form should also be completed and returned to Alexforbes.

Death claim form - death of a member's spouses ON OR AFTER 1 APRIL 2025 (Momentum form)

This Momentum form should be completed to notify the Fund of the death of a member's spouse (while the member is in office), if death occurred on or after 1 April 2025.  There is also (included) a Fund form confirming the number of spouses the member had, at the time of the spouse's death.  The completed forms plus supporting documents should be sent to zzpobpen@alexforbes.com .  

Death claim form - death of a member ON OR AFTER 1 APRIL 2025 (Momentum form)

This Momentum form should be completed to notify the Fund of the death of a member while in office, if death occurred on or after 1 April 2025.  The completed form plus supporting documents, together with the Alexforbes form, should be sent to zzpobpen@alexforbes.com .  In the event of the death of a member, the Trustees will carry out an investigation into the family circumstances (as required by the Pension Funds Act) - please see "Guide to the allocation of death benefits" for more information.

Change of Banking Details form

The duly completed and signed form, plus the supporting documents requested in the form, should be e-mailed to zzpobpen@alexforbes.com 

In-Fund Living Annuity - declaration form

This form should be completed and returned to zzpobpen@alexforbes.com if you are choosing to invest part or all of your retirement benefit in the In-Fund living annuity option.

To claim a share of your ex-spouse's Fund benefit, after divorce.  The completed form should be sent to zzpobpen@alexforbes.com
